This one-day colloquium brings together senior leaders and safety professionals to examine how culture, governance and decision-making affect the management of catastrophic incident risks. The event features presentations by Trish Kerin alongside peer discussion and exploration of real-world case studies.
Three major incident case studies will be presented through video, with detailed analysis of their causal factors and organisational preconditions. Participants will then reflect on and discuss these examples in depth, working to understand the deeper systemic causes behind significant incidents. The colloquium culminates in shared discussion of practical strategies for improving how culture, leadership and governance can support robust decision-making and long-term risk management.
By the end of the day, participants receive notes on the case studies and guidance on recognised good practice approaches that address real-world leadership challenges.
